Thank you for your interest to participate in SSA Program. Please complete the following questionnaire to determine how your department needs align with the product offering and to provide most appropriate level of support.

Developer License is required to develop and publish Dashboards and Data Sources
Please provide details of the licenses you own. For example, Desktop License, Creator License, Sever License etc.
Interactors can edit already developed dashboards or create their own dashboards connecting with the data sources published by the Developer
Viewers can view and filter already developed dashboards but, cannot edit or create their own dashboards
Data Source Details
Provide the types of data sources you will be connecting to. For example: Oracle, SQL Server, Excel, Web Data etc
Provide the business nature of your data (i.e. Financial, Clinical, Quality, Education, Research etc.)
Please select the classification type of your data: P1, P2, P3 or P4 To better understand UCSF data classification types, please visit: https://it.ucsf.edu/policies/dataclassification
Provide the name of the person who owns the data
Do you currently have approval from the Data Owner to use the data?
