Step 9.1 - Submit Tableau License and Access Request Form (LARF) for Interactor or Viewer Access in Tableau QA Site

Tableau License and Access Request Form is required to request "Interactor" or "Viewer" access on Tableau QA Site. Tableau Developer from the concerned department is responsible to submit the LARF.

Instructions

1. Access Tableau License and Access Request form (LARF) using following link: Tableau LARF

2. Fill out the "Requester Info" section as shown below. Select "Requester Organization" as either Campus or Health. 

3. Fill out the "Account Request" section as shown below. Select the Type of Role of your choice as either Interactor or Viewer.

4. Fill out the "Requested For" section as shown below. Please note that, you can request upto '10' users in a single request. In the below example, we are requesting access for two users. If you want to include third user to the request, simply add it in the fields highlighted in the red border. 

5. Provide proper "Business Justification" and select the "Purpose" of the use

6. Select the "User requires access to QA for testing purposes" and select the corresponding Projects from the available list. 

7. If the request is for P4 dashboards, make sure to attach special approval from the Senior Vice President of your department. 

8. Submit the request.

9. Once you submit the request, ServiceNow sends a notification to your manager to approve the request. Developer is responsible to follow up with manager to get approvals.

10. Once the request is approved, Interactors/Viewers may receive an automated e-mail to complete DocuSign attestation, if the "Data Classification Type" of your "Project" is P3 or P4 or the "Purpose" is ClinicalTreatment, Paymentand Operationor Quality Improvement/Quality Assurance or Research.

11. Interactors/Viewers complete DocuSign attestation. 

12. Tableau Admin team will setup their account and grants access to the corresponding projects on "Tableau QA Site" 


FAQs

1. How long does it take to grant access?

It usually takes '1' to '2' business days once the request is Approved, SVP Approval is atatched (P4 dashboards only) and DocuSign attestation is completed

2. What is the difference between Interactor and Viewer role?

3. Does is cost any subscription or maintenance fee to apply for Interactor or Viewer access?
